North Cornwall Foot Care is committed to protecting your personal information, so to comply with General Data Protection Regulation (GDPR ) 2018, this policy will describe how North Cornwall Foot Care collects, uses and stores information obtained. 

Information is collected either at an appointment, phone enquiry or via the website contact button.

In this notice, reference to "we" or "us" means North Cornwall Foot Care

Information we collect:

During the first consultation, we will collect personal information such as name, date of birth and contact details. We will also ask you about your general health, medical conditions and any allergy or medication requirements.

Any information is recorded on patient notes in paper form. These are held in a locked filing cabinet. We do not hold or record any medical data on digital form. i.e. computers, tablets, mobile phones etc

This information maybe updated on

future visits if required and appropriate.

How we may use your information:

With your agreement, we may use your details to liase with other Health Care Professions, if required and necessary for you care and ongoing treatment.

We do not share or sell any of your information with 3rd parties .

Your Rights:

You have the right to request a copy of any information we hold about you. This can be obtained by submtting a written request to:

Keeping your records:

We have a legal requirement to retain your personal information for a minimum of 7 years after the last treatment. After this time your information will be safely destroyed, as per medical regulations. If you are under the age of 18 when treated, the records will be kept until your 18th birthday.

Changes to this notice:

If we decide to change this GDPR privacy statement, we will post the changes here.

If we make any significant changes to the way we treat your personal information we will contact you directly.
